All employees enrolling in a PHCC Medical Insurance Trust medical plan are automatically enrolled for $10,000.00 in Life and AD&D insurance coverage paid for by the Employer at the low cost of $2.80 per employee, per month.

Term Life: This coverage is Term Life Insurance. The life insurance is payable to the designated beneficiary upon death of the insured. Coverage is provided for the time period that the employee is employed and premium is paid. There is no cash value associated with this product.

AD&D: Accidental Death and Dismemberment Insurance provides specified benefits for a covered accidental bodily injury that directly causes dismemberment. In the event that death occurs from a covered accident, both the Life and AD&D benefit would be payable.

Voluntary Life Insurance: Employees may also purchase additional group term life insurance on a payroll deduction basis. They may choose either $25,000 or $50,000 guarantee issue (no medical underwriting). In addition, they may also cover their spouses for up to 50% of the benefit of the employees coverage, and cover their children for $10,000 each.
